If you already have a cage attached to a fixed ladder osha will accept its use for the next 19 years.
Inside widths of wells must be at least 30 inches 76 cm.
Multi section ladders with a climb of 24 feet or more require rest points.
For a parapet ladder the access level shall be the roof if the parapet is cut to permit passage through the parapet.
Bottoms of wells above the point of access.
Intermediate structural members for example midrails or screens must be installed when there is no wall or parapet wall at least 21 inches high 29 cfr 1926 502 b 2.
Wells must completely encircle the ladder.
The new regulations change this requirement dramatically.
Under the new regulations a ladder over 24 feet high will require a personal fall arrest system or ladder safety system.
Per osha 1910 23 b 4 ladder rungs steps and cleats have a minimum clear width of 16 inches measured before installation of ladder safety systems for fixed ladders.
Section 1910 28 establishes the requirements that employers must follow on the use of cages and wells as a means of fall protection.
You can choose any pfas provided it meets the osha requirements in section 1926 502 d.
Osha is giving general industry workplaces including warehouses utilities retail and manufacturing some time to transition.

Wells must be free of projections.
A ladder that is less than 24 feet high does not require a fall arrest system of any sort.
These are meant to protect climbers as they ascend.
Platforms used with fixed ladders provide a horizontal surface of at least 24 inches by 30 inches 61 cm by 76 cm.
For through ladders the steps or rungs are omitted from the extensions and the side rails are flared to provide not less than 24 inches 61cm and not more than 30 inches 76 cm of clearance.
Previously a fixed ladder with a cage required a landing platform at a maximum interval of 30 feet.

Inside faces of wells on the climbing side of the ladder must extend between 27 inches 68 cm and 30 inches 76 cm.
As of november 19th 2036 cages will no longer be accepted as a form of fall protection and all fixed ladders taller than or that extend beyond 24 feet high must use a personal fall arrest system or a ladder safety system.
The top rails of a guardrail system must be 39 to 45 inches above the walking working surface 29 cfr 1926 502 b 1.
